شناسایی فرآیند بهبودی نشانه‌های افسردگی در روان‌درمانی پویشی کوتاه‌مدت فشرده: یک مطالعه تک آزمودنی

چکیده
هدف پژوهش حاضر، شناسایی فرآیند بهبودی نشانه‌های افسردگی در روان‌درمانی پویشی کوتاه‌مدت فشرده بود. طرح پژوهش از نوع آزمایشی تک آزمودنی با چند خط پایه‌ و دوره پیگیری بود. جامعه آماری شامل سربازان وظیفه مبتلا به اختلال افسردگی اساسی یک واحد نظامی در اصفهان در بهار سال 1403 بود. 3 نفر به روش نمونه‌گیری هدفمند انتخاب شدند. شرکت‌کنندگان پروتکل ISTDP دوانلو (1995) به روش فردی طی 8 جلسه 90 دقیقه‌ای به‌صورت هفتگی دریافت کردند. شرکت‌کنندگان، با نسخه دوم پرسشنامه افسردگی بک در طی 12 مرحله ارزیابی شدند که شامل 4 مرحله خط پایه (4، 3، 2 و 1 هفته پیش از اولین جلسه درمان)، 4 مرحله مداخله (پس از اتمام جلسات درمانی دوم، چهارم، ششم و هشتم) و 4 مرحله پیگیری (1، 2، 3 و 4 هفته پس از آخرین جلسه درمان) بود. تجزیه‌وتحلیل داده‌ها با استفاده از تحلیل بصری نمودار، درصد بهبودی و شاخص تغییر پایدار انجام شد. نتایج نشان داد، هر سه شرکت‌کننده با شروع مداخله، روند کاهشی و رو به بهبودی را در تراز نمرات افسردگی نشان دادند و پایداری این نتایج تا یک ماه پس از درمان تایید شد. بر اساس نتایج پژوهش پیشنهاد می‌شود که از ISTDP برای بهبود بیماران مبتلا به افسردگی استفاده شود.

The objective of this study was to explore the process of symptom improvement in patients with depression undergoing Intensive Short-Term Dynamic Psychotherapy (ISTDP). The research employed a single-case experimental design with multiple baselines and follow-up assessments. The participants were conscripted soldiers diagnosed with major depressive disorder from a military unit in Isfahan, during spring 2024. Three participants were selected through purposive sampling and individually received the ISTDP protocol based on Davanloo’s (1995) method in 8 weekly 90-minute sessions. Assessment using the Beck Depression Inventory-II (BDI-II) was conducted across 12 time points: 4 baselines (1, 2, 3, and 4 weeks before the initial session), 4 interventions (post sessions 2, 4, 6, and 8), and 4 follow-up stages (1, 2, 3, and 4 weeks post-treatment). Data analysis involved visual charting, percentage improvement, and Reliable Change Index (RCI) measures. Results indicated a downward trend in BDI-II scores beginning at intervention, with improvement sustained through one-month follow-up, supporting ISTDP as a viable treatment option for depression.
